This video is presented to you by the International Students and Scholars Office to help you follow the I-765 Employment Authorization Form and G-1145 E-Notification application accurately. These forms are commonly used for Optional Practical Training (OPT) and extension employment authorization. Form I-765 is used to request an Employment Authorization Document (EAD) and receive an employment card. When filling out the form, be sure to sign the applicant signature section correctly. The fillable version of Form I-765 now includes reminders to sign the form and arrows pointing to the signature section. Remember to sign the form before mailing it, as USGS will reject any unsigned form and delay the processing of the application. It is important that the signature does not go over the signature lines as shown. The ISS staff anticipates that you will have this form filled out completely prior to your OPT appointment. Form G-1145 is used to request text message and/or email notifications when USCIS accepts your form. Do not include any personal information, including your name, as email and text message are not secure ways to transmit information. The notification will provide your seat number and instructions for obtaining case status information. To access Form I-765, visit our website and go to the forms menu under OPT and STEM. Make sure you are filling out the latest form by checking the expiration date in the top right corner. We strongly recommend typing the form online instead of writing it by hand. For students applying for OPT STEM, select "Permission to accept employment" and check the for "Renewal of my permission to accept employment".
